TITLE: EMT TESTING FEES POLICY STATEMENT: The EMS Systems shall collect the appropriate testing fee from each EMS student and submit one certified check or money order to the Illinois Department of Public Health for the total amount of the testing fees from each course instructed by the System. The EMS Systems shall maintain records showing collection of testing fees when the fees are retrained by the Resource Hospitals. GOAL/PURPOSE: To insure the proper process is followed in the collection of EMS Student testing fees and the payment process for State examination fees. POLICY/PROCEDURE: A. A candidate for an EMT State written examination shall submit the required fee in cash, money order or certified check made payable to the respective EMS System Hospital sponsoring the course. The testing requirements are;
B. The respective EMS Resource Hospital shall collect all fees and verify payment for all candidates testing on a specific date and location. C. The respective EMS Resource Hospital shall submit to the Illinois Department of Public Health one certified check or money order for the total amount of the testing fees from each course instructed by the System. D. The respective EMS Resource Hospital shall maintain records showing
collection of testing fees when the fees are retained by the Resource
Hospitals. These records shall be maintained by the EMS System Coordinator
and available for inspection by the Department at any time. |
