TITLE: EMT LICENSE RENEWALS USE OF TRANSACTION CARD

POLICY STATEMENT:

Identical renewal requirements have been established and implemented by the BroMenn Service Area EMS System and the OSF St. Joseph Medical Center EMS System. Those individuals who have completed all requirements for re-licensure and is a member of either EMS System shall provide the appropriate documentation to their respective System no later than 30 days prior to the licensee expiration date. The respective EMS Medical Director shall submit a "Transaction Card" to the Department to recommend re-licensure of the licensee. Specific procedures have been established in this policy to provide direction when an EMT desires a voluntary reduction of an EMT status or to reinstate status to a more advanced level. In addition, this policy includes the procedure for renewal of an EMT license that has bee expired for a period of less than 60 days and the procedure for the use of the "Transaction Card" by the EMS System.

GOAL/PURPOSE:

To provide additional direction to EMS providers in the process of EMT renewal.

POLICY/PROCEDURE:

A. Those individuals who have completed all requirements for re-licensure and is a member of either EMS System shall provide the appropriate documentation to their respective System no later than 30 days prior to the licensee expiration date. The respective EMS Medical Director shall submit a "Transaction Card" to the Department to recommend re-licensure of the licensee.

B. If the EMS Medical Director does not recommend re-licensure, he/she shall submit all reasons for denial in writing to the EMT and the Department.

C. The license of an EMT who has failed to file an application for renewal shall terminate on the day following he expiration date shown on the license.

D. Voluntary status change from and EMT-I to an EMT-Basic, or EMT-P to an EMT-I or EMT-Basic:

- At any time prior to the expiration of the current license, an EMT-I may revert to the EMT-B status or an EMT-P may revert to the EMT-I or the EMT-B status for the remainder of the license period. The EMT-I or EMT-P must make this request in writing to the EMS System. This request along with the written approval of the EMS Medical Director, a completed transaction card by the EMS System indicating the voluntary reduction and the original license of the individual shall be forwarded to the Department for final determination. To re-license at the reverted level, the individual must meet the active status re-licensing requirements of that level.

E. An EMT-I or EMT-P who has reverted to EMT-B status may be subsequently re-licensed as an EMT-I or EMT-P, upon the recommendation of the EMS Medical Director who has verified that the individual's knowledge and clinical skills are at an active EMT-I or EMT-P level, and that the individual has completed any retraining, education or testing deemed necessary by the EMSMD for resuming EMT-I or EMT-P activities. After verification of the individual's knowledge and clinical skills, the EMSMD shall submit a written recommendation and completed transaction card reinstating the individual to the advanced level to the Department for final determination. If approved by the Illinois Department of Public Health, a new license will be issued to the individual.

F. An EMT whose license has expired may, within 60 days after licensure expiration, submit all re-licensure materials as required for renewal to the EMS System office along with a certified check or money order for $50.00 made payable to the Illinois Department of public Health. If the renewal materials are in order and there is no disciplinary action pending against the EMT, the EMS Medical Director shall submit the $50.00 fee and a transaction card for re-licensure to the Department for final determination. If approved by the Illinois Department of Public Health, a new license will be issued to the individual.

G. Use of Transaction Card: All EMS System members shall submit the appropriate documentation for licensure or re-licensure to their respective EMS Medical Director for verification of fulfilling the requirements of licensure/re-licensure. After verification, the EMS Medical Director shall submit a "Transaction Card" (Form No. Il. 482-0837) to the Department. If approved by the Illinois Department of Public Health, a new license will be issued to the individual.
